A back tooth needs a crown after root canal treatment because the procedure removes the pulp and a significant amount of tooth structure, leaving the remaining walls thin and brittle. Without a crown to hold those walls together, normal chewing force can split the tooth vertically — a fracture that usually cannot be repaired and means losing the tooth. Front teeth with minimal damage sometimes do not need one.
What root canal treatment does to a tooth
To reach the canals we cut an access cavity through the biting surface. The pulp, which supplied the tooth with moisture and nutrients, is removed. Often the tooth had already lost substance to decay or fracture before treatment even began.
What remains is a hollowed shell. It looks intact from outside, and that is precisely the problem — the weakness is not visible.
Why molars specifically
Back teeth take the highest chewing load, and their cusps act like wedges under it. With the roof of the pulp chamber gone, those cusps flex outward with every bite. Repeated flexing produces a vertical root fracture, which runs down the length of the tooth.
A vertical root fracture is effectively terminal. It cannot be filled or bonded, and the tooth is extracted. Replacing it with an implant costs several times what the crown would have.
When a crown genuinely is not needed
- Front teeth (incisors and canines) with a small access cavity and otherwise intact structure — these take shearing rather than crushing forces.
- Teeth with very little remaining decay and no existing large filling.
Even then we review it, because a front tooth that darkens over time may need a crown or veneer for appearance rather than strength.
How long you can reasonably wait
- Immediately after treatmentA temporary filling seals the access cavity.
- Within two to four weeksThe permanent crown should be fitted. This is the target.
- Beyond three monthsRisk rises steadily. The temporary filling wears, leaks, and lets bacteria back into the canals — which can mean the whole root canal has to be redone.
If cost is the obstacle, say so
There are usually options — a different crown material, or staging the treatment. What does not work is quietly postponing it indefinitely and hoping. Tell us, and we will plan around the constraint rather than let the tooth fail.
Questions we get asked
Can I just leave the filling and skip the crown?
On a back tooth, this is the single most common reason a successful root canal ends in extraction. The filling seals but does not hold the cusps together.
How soon after a root canal can the crown be fitted?
Usually within one to two weeks, once symptoms have settled. There is no benefit to waiting longer and real risk in it.
Which crown material is best after a root canal?
Zirconia for most cases — strong, metal-free and good-looking. Metal-ceramic remains a reasonable lower-cost option for molars that are not visible.
